DEADLINE FOR RECEIPT OF FALL TERM REQUESTS IS
SEPTEMBER 12, 2008
Requests for classes with 100 or more students registered must be returned by September 5, 2008.
Surveys are scheduled as they are received; early submissions are more likely to be assigned the first choice of survey date. More than 3,000 surveys need to be scheduled and processed; please return yours promptly.
- Complete a request form by CLEARLY PRINTING all of the information requested. Make sure you indicate the number of students in each class. You may update the number up until the date of your survey by calling (412) 624-6147. Incomplete entries will delay the scheduling of your survey(s). Keep a photocopy of your request for reference.
- List three choices of survey dates from Wednesday, October 15 through Friday, December 5 in order of preference. Surveys also can be scheduled the week of December 8-12 for graduate classes not having a final examination that week. Make sure that these dates are on days your class meets and will not conflict with examinations. Surveys cannot be scheduled before 8:30 a.m. or after 8:30 p.m. No surveys will be given November 26-29.
- If you are teaching an online course, please indicate this and the number of students on the form. An online evaluation will be done.
- SIGN THE FORM!
- A sample of the survey form used in your school can be found here. It will be given when there are seven or more students present for the survey. A qualitative questionnaire will be given when five or six students are present. Classes with four or fewer students will not be scheduled.
- You may select up to 10 additional items from the additional items bank. List the item numbers in the spaces provided for each course you enter. The additional items bank can be accessed here.
